Due to the HIPPA privacy act, only you can officially transfer your records to South Lake Pediatrics. Please use our Release of Records Form. Once completed, please send it to your previous clinic and they’ll transfer your records to us.

Please bring our Demographic Form and your health insurance card to your first appointment with us.

Once you've completed your first visit with us, you will have the opportunity to utilize our Online Services which includes Online Appointment Scheduling and Bill Payment.
