Our Care Coordination Team is here to make the process easier, helping you explore therapy options and find the right support for your mental health journey.

Please call us at 952-401-8300 or send your questions to the patient portal CLICK HERE.

Connect with one of our therapy referral partners for priority access to behavioral and mental health services. To get started, review our list of community providers then submit your referral request via the patient portal. Once logged in, click Messages > Referral to a Specialist > Request Referral. Be sure to include the name of the agency requested and your best contact information. Once processed by our team, the community agency will contact you directly to arrange an appointment.
